HOSPITAL-INTEGRATED COMMUNITY HEALTH WORKER
NCDHD is seeking a full-time Hospital-Integrated Community Health Worker based at Cherry County Hospital in Valentine. This position works with patients and healthcare partners to connect individuals with resources, support chronic disease management, and improve access to preventative health services. A high school diploma or GED is required, and NCDHD offers a competitive benefits package.
